What to expect at your visit
1 Wear comfortable clothes and bring any required forms, including a completed health and immunization history. Sign in at the electronic kiosk to see a practitioner. No appointment necessary.
2 Your practitioner will perform an examination, including:
- Review of your health history
- Review of your immunizations to ensure they are up to date
- A simple physical exam to determine if you are able to safely participate in typical sports-related activities
- Provide sports safety and education tips
3 We'll give you a summary of your exam and stamp any medical forms required.
4 We also can send the summary to your primary care provider at your request.
5 You are responsible for the price listed on our menu. Insurance is not accepted for this service. Special $35 offer expires 9/19/10.
What to know before your visit
- Service is not available in Massachusetts.
- We recommend routine health evaluations with your primary care provider. Therefore, this physical exam requires cash payment and will not be submitted to your insurance since most plans will only pay for one routine physical examination per year.
- We do not offer "certificates of immunization." Patients should obtain copies from their primary care providers or from the clinics where they routinely receive immunizations. We can only certify/verify immunizations actually administered at MinuteClinic.
- We cannot refill prescriptions for conditions that are outside our scope of services. Patients should call their primary care provider to refill any needed prescriptions.
