What to expect at your visit
1 Wear comfortable clothes and bring any required forms, including the DOT Medical Examination Report with “Driver Information” and “Health History” sections completed. Sign in at the electronic kiosk to see a practitioner. No appointment necessary.
2 Your practitioner will perform an examination, including:
- Review of your health history
- Urine testing
- A simple physical exam
3 We’ll complete your Medical Examination Report for Commercial Driver Fitness Determination form, give you a summary of your exam and provide a Medical Examiner’s Certificate as appropriate.
4 We also can send the summary to your primary care provider at your request.
5 You are responsible for the price listed on our menu. Insurance is not accepted for this service.
What to know before your visit
- Service is not available in Massachusetts.
- We recommend routine health evaluations with
your primary care provider. Therefore, this physical exam requires cash
payment and will not be submitted to your insurance since most plans
will only pay for one routine physical examination per year.
- We cannot refill prescriptions for
conditions that are outside our scope of services. Patients should call
their primary care provider to refill any needed prescriptions.
