Diabetes is a leading cause of disability and death in the United States. High blood glucose is one risk factor that can increase your chance of developing — or suffering complications from — diabetes.

A screening can help identify high blood glucose (sugar). And that can help you prevent or delay the effects of diabetes.

What to expect at your visit

1 We recommend that you fast 8 to 12 hours before coming in. Then sign in at the electronic kiosk to see a nurse practitioner or physician assistant. No appointment necessary.
2 Your practitioner will perform a risk assessment. A blood sample will be taken from your finger to test your blood glucose (sugar).

If you already have diabetes, we will test your glycosylated hemoglobin (A1C).

3 We’ll discuss your results and recommendations for risk reduction.
4 We’ll give you a visit summary and send it to your primary care provider at your request.

What to know before your visit

  • Service is not available in Massachusetts, Nevada, New York and Pennsylvania.
  • Patients must be 18 years of age or older.
We employ both physician assistants and nurse practitioners in Minnesota, Nevada, North Carolina and Texas.
Open 7 days a week even on evenings and weekends
