Cardiovascular disease, diabetes and stroke are leading causes of death in the United States. Several behaviors and conditions (risk factors) can increase your chances for developing – or suffering complications from – these serious health problems.

A comprehensive health screening can help identify risk factors. And that can help you prevent or delay the effects of these diseases.

What to expect at your visit

1 We recommend that you fast 8 to 12 hours before coming in. Then sign in at the electronic kiosk to see a nurse practitioner or physician assistant. No appointment necessary.
2 Your practitioner will perform a risk assessment. He or she will take a blood sample from your finger, then perform the following tests:
  • Cholesterol
  • Diabetes (blood blucose or A1C)
  • Hypertension (blood pressure)
  • Weight evaluation
3 We'll discuss your results and recommendations for risk reduction.
4 We'll give you a visit summary and send it to your primary care provider at your request.

What to know before your visit

  • Service is not available in Maryland, Massachusetts, Nevada and Pennsylvania.
  • Patients must be 18 years of age or older.
